New Patient Center

New Patient Checklist:

1. Insurance Card- if using insurance, we ask that you please bring all current vision and medical insurance cards associated with the patient. We will also request a photo ID of the patient or parent/guardian.

2. Glasses and Contacts- if currently wearing glasses, we ask that you please bring them with you. If currently wearing contacts, we ask that you also bring the boxes that your lenses came in.

3. Medication Lists- if currently taking medications we would appreciate a list of all current medications, including the name and dosage.

If possible, we also accept records from previous offices to better serve you.
